Listeriosis with terminal ileitis and bacteremia in a healthy 9-year-old boy

Listeriosis, caused by Listeria monocytogenes, leads to severe infections, such as sepsis and meningitis, in newborns, pregnant women, older adults, and patients with cell-mediated immune disorder. On the other hand, it rarely infects healthy children without cell-mediated immune disorder. We encountered a case of listeriosis that developed in a previously healthy 9-year-old boy. The patient presented with fever, abdominal pain, and diarrhea symptoms that persisted for five days after consumption of processed meat. On performing a stool culture, only Staphylococcus aureus was detected; further, abdominal ultrasonography revealed terminal ileitis accompanied by ileocecal lymphadenitis; therefore, yersiniosis was suspected, and ceftriaxone were administered, after which, the patient’s condition improved. However, subsequent blood test results revealed the presence of L. monocytogenes, and the patient was diagnosed with listeriosis. It is thus suggested that the imaging findings of terminal ileitis accompanied by ileocecal lymphadenitis, in addition to detailed medical history-taking and blood culture results, may be useful for the diagnosis of listeriosis.
