If you need to change your registered information such as workplace, family name, or mail address, please inform us by using a registration matters form (Japanese Only) or update it by yourself.
Withdrawal notification
Fill out the Withdrawal notification form (Japanese Only) and send it to our Member Support Desk via e-mail (E-mail only).
Reminder: If you are certified as an Infection Control Doctor (ICD) by our recommendation, you will lose the certification when you lose the membership.
