Recurrent subcutaneous abscess caused by Panton-Valentine leukocidin positive methicillin-resistant Staphylococcus aureus:a case report

Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) is an MRSA clone, which was reported recently as an organism causing skin and soft tissue infection in healthy children and young adults. It is different from hospital-acquired MRSA (HA-MRSA) and, in particular, Panton-Valentine leukocidin (PVL)-producing MRSA, which is highly pathogenic. A 2-years-old healthy girl visited this hospital because of repeated abscesses on her hips, and MRSA was detected in the abscess culture. Some abscesses occurred on her hips after her hospital visit. In addition, her brother and mother also developed a small skin abscess. Sulfamethoxazole/trimethoprim and rifampicin were used together for the patient, and her family members were treated similarly at the same time. They were all educated about the prevention of contact infection, and after that, all of the abscesses improved. No immune abnormality indicating susceptibility to staphylococci occurred. The MRSA detected in the patient’s abscess culture was analyzed by molecular epidemiological methods. The SCCmec type was detected as Ⅳ a, and the PVL-producing gene was positive. Multilocus sequence typing (MLST) revealed a sequence type of ST8, indicating this strain as potentially a USA300 clone.
