Acute rheumatic fever in which emm type 89 group A Streptococcus was isolated from throat culture; a case report

There has been a marked decline in the incidence of acute rheumatic fever (ARF) in most developed countries. However, a small outbreak of ARF was reported recently in the USA. Group A Streptococcus (GAS) pharyngitis outbreaks caused by certain emm types that are highly associated with ARF. A 6-year-old healthy Japanese boy was referred to this hospital by his primary care physician for the evaluation and treatment of arthralgia and fever. Based on the presence of one major Jones criterion and three minor ones, he was diagnosed as ARF. GAS emm type 89 was isolated from a throat culture. The boy recovered after treatment with oral nonsteroidal anti-inflammatory drugs and antimicrobial agents. It is believed necessary to study the association between GAS emm types and ARF in Japan, because this may be helpful in predicting future ARF outbreaks.
