A case of acute encephalopathy with biphasic seizures and late reduced diffusion associated with respiratory syncytial virus infection

A male case of acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) associated with respiratory syncytial virus (RSV) infection was presented. He had fever at the onset, accompanied by convulsions for several minutes. The patient experienced weakness of the left upper extremity, convulsions and disturbance of consciousness on the fourth day of illness. Late diffusion-weighted imaging from a brain MRI showed high intensity in the frontal lobe subcortical white matter. Elevated levels of IL-6 and tau protein within the cerebrospinal fluid suggested the presence of encephalopathy, and dexamethasone and intravenous anti-epileptic medication were introduced. RSV genome was detected by the RT-LAMP method in the cerebrospinal fluid. The patient's left monoplegia protracted, but recovered without sequela.
