Soft tissue infection can be life threatening if the diagnosis and treatment are delayed especially when a deep structure is involved. Therefore, such deep soft tissue infection requires prompt surgical treatment and/or broad spectrum antibacterial drug. We experienced a case of anaerobic cellulitis in 1 year 9 month old female infant with an unknown cause of injury and no evidence of underlying disease in the right thumb. The patient's right thumb was partly black, and an X-ray showed a gas shadow in the lesion. Since gas forming soft tissue infection is clinically difficult to diagnose at the first presentation, urgent surgical intervention was considered to be important for differential diagnosis particularly when a life threatening disease was suspected.
