The patient was a seven-year-old girl who presented with an encephaloma, in whom a brain tumor was suspected and a craniotomy enucleation was performed. However, the histopathological findings of the resected specimen were not consistent with a brain tumor, and the brain lesion was diagnosed as cerebral paragonimiasis caused by Paragonimus westermani by using immunological ELISA. This case underscores the importance of bearing in mind cerebral paragonimiasis in the differential diagnosis of encephalomas associated with the symptoms and imaging findings of brain tumor such as headache, vomiting, convulsions, etc. A careful and detailed diet and travel history should be obtained, with examination of the peripheral blood eosinophil count, serum IgE levels, etc.
